Woods Primary and Behavioral Health Center at Hamilton (Woods at Hamilton) will be the first of its kind in the area to offer integrated primary care and behavioral health services to individuals with intellectual and developmental disabilities, autism, neurodiversity and complex healthcare needs. With integrated care, all of an individual’s providers work together to provide seamless communication and whole-person care. This improves health outcomes, increases convenience, and reduces costs.
